Independent Health
Independent Health has elected to offer a single plan for their Chamber of Commerce clients in 2010. Independent Health will continue to offer their Flexfit Select plan options including Flexfit Active, Flexfit Independent, and Flexfit Family. If you would like to participate in Independent Health’s Flexfit Select offering and are not currently enrolled in that plan, you will be required to complete a new application. As with the other carriers, if you are moving to Independent Health from another carrier, you will be required to follow the underwriting guidelines set forth by Independent Health. Health Insurance coverage through Independent Health requires completing each form listed below as well as following up with the forms requested on the Employer Certification Form.
